Here are some forms that you may find useful during your child’s preschool education with Little Tree House Group. Please note that IT IS YOUR RESPONSIBILITY to inform Little Tree House Group in writing of your intentions of any changes. The reason is logically and reasonably simple – our staff are not mind readers and are unable to read your mind or know what you and your family are experiencing at any point in time! As a responsible entity, we are always transparent and upfront on issues, especially with it comes to care of your child and financial aspects (terms and conditions are spelt out clearly during your child’s registration).
 
 
APPLICATION FOR ADMISSION INTO INFANT CARE OR CHILDCARE CENTRE AND SUBSIDY APPLICATION
Use this form – Form 1: Application for Admission to Infant Care or Child Care Centre Cum Subsidy Application if you wish to enrol your child with Little Tree House’s infant care or child care programme and/or apply for subsidy. Please note that subsidy is applicable for Singapore Citizens only (amount and approval is subject to meeting certain criteria).
 
 
NOTIFICATION OF CHANGES
Use this form – Form 3: Notification of Changes if there are changes to your working status and/ or household income, change in child’s or main applicant’s or spouse’s nationality and change in programme type and programme fee during your child’s preschool education with Little Tree House Group.
 
 
CHANGE OF SUBSIDY APPLICANT
Use this form – Form 3A: Change of Subsidy Applicant if you changes to the subsidy applicant is needed.
 
 
WITHDRAWALS FROM CENTRE OR SUBSIDY
Use this form – Form 4: Withdrawal from Centre or Subsidy if you wish to withdraw your child or subsidies. Please note that withdrawals only take place on the last day of the month and notice of withdrawal has to be received by the centre in writing, at least 1 calendar month, from the last day of the month, in order for the deposit held with Little Tree House Group (LTH) to be refunded. (I.e. if you wish to withdraw your child in December, the centre has to receive your notice of withdrawal in writing by 01 December of the same month. If notice of withdrawal is not received by the first day of the withdrawal month, deposit will not be refunded. LTH reserves the right not to refund your deposit if your child’s account is not cleared (i.e. outstanding school fees, insurance, etc.).
